The Circle of Hope Wall of Honor is to Commemorate Loved Ones who have been Affected by Cancer
 

You can submit your own tribute honoring your loved one who is or has battled cancer, or someone who has supported your during your own journey through the treatment process. You can honor a loved one who has passed, a loved one who is undergoing or has undergone treatment, or a loved one who supported you during your own treatment process.

 

For a $5 donation, your tribute will be featured on this page. For a $8 donation, your tribute will be featured on this page and a Facebook cover photo designed specifically to your tribute will be sent to you by email. Proceeds will be used to further Circle of Hope's mission in assisting uninsured and underinsured individuals with cancer, their families, and the community.

 

  • You will be able to select the message to be "In memory of," "In honor of," or "In recognition of."

  • You have up to 100 characters in personalized text.

  • You have to option to submit a photo of the individual you wish to honor. Only JPEG, JPG, PNG, GIF, TIFF, or PDF files will be accepted.

  • If a picture is not submitted, a pink ribbon will be used in its place. Photos will be sized to fit template.

  • You will receive an email notifying you once your submission has been uploaded to the Wall of Honor.

  • Facebook cover photos will be emailed to the address supplied.

  • Submissions remain online for one calendar year.

  • Please allow for one week for tribute to be processed.

 

After you complete the form below, you will be redirected to another page to complete submission. Completion of donation form is the final step in your submission.

 

Circle of Hope maintains the right to edit or not include any text that may contain profanity, explicit content, or any other inappropriate text. Circle of Hope also reserves the right to decline images that are explicit, inappropriate, or otherwise contradictory to the integrity of the project.The Wall of Honor is not to be used as a means of promotion or advertising of any sorts. All submissions are at the discretion of Circle of Hope.
